AFFIDAVIT OF CORRECTION

I, ROMANITO M. ACEBEDO, Filipino, of legal age, married, and a resident


of Buenavista, Tandag City, after having been duly sworn to in accordance with law,
hereby depose and say:

1. That in my Bureau of Internal Revenue Taxpayer’s Identification (TIN) Card,


my birth date was erroneously indicated as August 13, 1958;

2. My Birth Date is August 12, 1959 which is the true and correct date as
reflected in my Certificate of Live Birth;(Attached is the Certificate of Live
Birth as Annex “A”)

3. That I personally declare that my birth date is August 12, 1959;

4. That said erroneous entry was inadvertent error on the part of the informant
who relayed the information without intention to evade obligations or violate
any laws of the land;

5. That I am executing this affidavit to personally request the Bureau of Internal


Revenue to cause the reissuance of my correct birth date in my BIR Tin Card
and to apprise the authorities concerned on the veracity of the foregoing facts
and circumstances and for whatever legal purpose this may serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this March 25,
