Dear Mr/s. ______________:

We write this notice in relation to your employment as (Employee’s Position) of


(Company) to give a written explanation on the following matters:

1. Poor performance during (please insert the time period);

2. Gross and habitual neglect of duties; and

3. Failure to meet the standard of work expected by the Company for your position.

You are given three (3) working days from your receipt of this notice to give a
written explanation as to why no disciplinary action should be imposed upon you.
Depending on your explanation, we will will be constrained to impose the appropriate
penalty or sanction upon you. If you fail to submit your written explanation, the
Company will decide on this matter using what evidence it may have at hand and will
take your non participation and/or submission of defense as a sign that you are not
interested in submitting any defense.
