AFFIDAVIT OF HEIRSHIP WITH CONFIRMATION OF


SETTLEMENT

I, _____________________, of legal age, Filipino, single and a


resident of ______________________, Philippines, after having been duly
sworn to in accordance with law, do hereby depose and say, THAT:

1. I am one of the children-heirs of the late


_________________________ who died on February 13, 2012 in
Davao City, Philippines;

2. Being the children-heirs of the above-mentioned deceased, we


executed an Extra-Judicial Settlement of Estate dated June 16,
2012 acknowledged before Atty. ________________, a Notary
Public. Our deceased brother, _______________, was represented
by his children __________________________;

3. Since the estate of our aforesaid deceased brother has not yet been
settled by his legal heirs, I am requesting the Office of the Registry
of Deeds of Davao del Norte to issue title in his name
(___________________) as to his share in TCT No.
______________ pending settlement of his estate;

4. I hereby confirm the existence and due execution of the Extra-


Judicial Settlement of Estate mentioned in paragraph 2 hereof;

5. I am executing this affidavit to attest to the truth of the foregoing


facts and for whatever lagal purpose it may serve.
