Student Health Service
. We would like to invite your assistance in the enrolment exercise for the school year 2020/2021. In
view of the latest development of COVID-19, our health centres need to adopt social distancing and implement
infection control measures, we can only offer health assessment for the S1 students in 2020/21. If the situation
improves, we would consider arranging health assessment for other grades.
Each enrolled student will be scheduled for an appointment for health assessment at a designated
Student Health Service Centre according to the location of the attending school. Name of centre is designated for
your students. Its address is address of centre. The allocation of centre and health programmes for health
assessment may be adjusted owing to the latest situation of COVID-19. Appointments will be arranged from
November 2020 to October 2021. We should be grateful if you could distribute a set of documents consisting of a
“Letter to Parents/Guardians”, a “Notes for Parents/Guardians”, an “Application & Consent Form” and a pamphlet
(pamphlet will be provided to secondary one in 2020/2021) in your school. A sample of the set of documents is
enclosed as Appendix A. The documents for your school are now ready for collection. Please contact the staff of
name of centre at telephone no. of centre beforehand and ask your staff to bring along the completed “Collection
of Enrolment Documents” form (Appendix B) for collection during service hours.
Students joining the Student Health Service will be charged according to their eligibility status. The
service is provided free of charge to students who are “eligible persons”. Please see “Notes for Schools” (Appendix
C) for details. Kindly assist in completing the “Student Eligibility List” (Appendix D).
We would appreciate it if you could return the completed “Application & Consent Forms” from
parents/guardians (including those who do not agree to enrol), “Student Eligibility List” (Appendix D), “Enrolment
Summary” (Appendix E) and a school calendar to the name of centre by 14 September 2020. Please also advise us
on the dates we should avoid in fixing appointments for your students. Appointment letters will be sent to you for
distribution to parents about one month before the appointment date. Details of the enrolment procedures are given
in the “Notes for Schools”.
