having been sworn in accordance with law, hereby depose and state:

We personally know CLARENCE DELA CRUZ BALLIER,


being friends since we we’re young;

It was erroneously entered in his Certificate of Live Birth that his


first name is CLARINA and his sex is MALE ;

We declare and certify that his real name is CLARENCE DELA


CRUZ BALLIER and not CLARINA DELA CRUZ BALLIER, as
evidenced by his Certificate of Live Birth. A copy of his Voter’s
Identification Card and Baptismal Certificate issued by Parish of Sta.
Monica proving that he is using CLARENCE DELA CRUZ, is attached
hereto as Annex “A” and “B” respectively;

Moreover, we declare that his sex is MALE and not female;

He has been using the name CLARENCE in all his pertinent


documents and transactions;

We are not related by affinity or consanguinity to CLARENCE


DELA CRUZ BALLIER; and

We are executing this Affidavit to attest to the truth of the


foregoing statement and for whatever legal purpose it may serve.
