I, GENELYN GENIZA GAYAPA of legal age, married, and resident of


Camangahan, Guimbal, Iloilo after having been sworn to in accordance with law,
hereby depose and state:

said vehicle to park the same in our garage I accidentally hit the post of our
main gate, a portion of our gate and our neighbor’s bamboo fence;

3. That because of said incident the said motor vehicle incurred a dent
and/or damage on the left hand front fender, left hand front door, cowling side
RR, left hand lower front pillar, front door open, left hand guard door splash,
rear bumper, left hand front splash, left hand front door and open stopper body;

4. That I execute this affidavit to attest to the truth of the foregoing facts
and for whatever legal purpose it may serve.
