Acknowledgment of Service Form Guide

This document is an acknowledgment of service form used in family law cases. It has three parts: Part A is for information about the person being served, Part B is for listing which documents are being served, and Part C is for acknowledging receipt of the served documents. The form must be completed by the server in Parts 1 and 2 before being sent to the person being served. The served person or their lawyer then signs Part C to acknowledge receiving service of the listed documents on a specific date.
