AFFIDAVIT OF LATE FILING

I, JOSEPH SIBUANO SIBUNGA, of legal age, married, Filipino and


with postal address at Blk. 6 Lot 35 Brgy. San Miguel I Dasmarinas City, after
being duly sworn in accordance with law, do hereby depose and say:

1. That I am the bona fide holder of a document entitled DEED OF


LEGITIMATION OF NATURAL CHILD (LEGITIMATED BY SUBSEQUENT
MARRIAGE) executed by JOSEPH SIBUANO SIBUNGA and FRENCIE
MANUEL SIBUNGA in favor of SOPHIA BIANCA MANUEL dated January
22, 2020

2. That despite said facts of the said documents, I failed to file her
Certificate of Live Birth with and it was late to know that it was not forwarded
in the Local Civil Registry of Manila for registration purposes due to lack
and incomplete documents I wasn’t able to come back immediately;

3. That as a consequence, no Annotation on DEED OF


LEGITIMATIONOF NATURAL CHILD (LEGITIMATED BY SUBSEQUENT
MARRIAGE) could issued in our favor;

4. That I am executing this affidavit to attest to the truthfulness of the


foregoing statements and for all legal intents and purpose it may serve.
