5. Certified photocopy of General Weighted Average in the Baccalaureate course
6. Certified copies of Transcript of Records for Baccalaureate course
7. Certified photocopy of Certificate of Registrar as to the MA units/degree earned
8. Certified copies of certificates of trainings and recognitions
9. Certified photocopy of Voter’s Certification/Voter’s ID
10. Certified photocopy of NBI clearance
11. Copy of Certificate of Employment
12. Omnibus of certification of authenticity and veracity of all documents submitted, signed by
the applicant.
