HEALTH & SAFETY PLAN Rev 1

1. PURPOSE

The purpose of IHCC Construction Health, Safety and Environmental Management


System is to prevent injury, loss of life, and damage to environment and assets. The
program provides safety criteria for all construction work performed by contractor itself
and its sub-contractors.

The requirements in this plan detail the need for client services or equipment. The
contractor shall provide equivalent equipment or services from their own resources and
also to ensure that activities, conducted by its personnel or sub-contractor are in
accordance with client requirements and all applicable international, national and local
rules, standards, regulations and requirements are followed.

The plan reflects the latest client and construction industry practices. For project to
proceed safely, all contractor and sub-contractor personnel involved in construction
activities, whether management or site staff will follow the provisions in this plan. Only
with the active commitment of everyone we ensure that the safest possible work
environment can be maintained.

2. IHCC SAFETY

IHCC Construction Co. is fully committed to undertake its business in such a way as to
minimize the risk of injury, damage to property or the environment. We have moral &
legal obligation to safeguard the health, safety and welfare of employees & interests of
customers who may be affected as a result of company's activities.

We fully accept obligations & responsibilities to work safely, to take all reasonable care
for employee's health & safety and to do all that is reasonably practicable to prevent
accidents, injuries & damage to health, including a commitment to continual
improvement in OH & S management & performance.

We are committed to comply with illegal & statutory requirements that are related to
OH & S risks. Providing & maintaining safe working environments & ensuring that all
employees play an active part in the health & safety programs & facilitate them with
adequate information, instruction, training & supervision.

We regularly improve & sustain the confidence level of existing & potential customers,
improve work safety conditions & strive to uphold core values of safety, knowledge,
integrity & leadership in order to achieve goal of zero harm.

PART II
4. HSE RESPONSIBILITIES

4.1 TQM Manager:


1. Assist Top Management for establishing H&S Policy and corporate objectives.
2. Ensure OH&S Management System implementation.
3. Produce and develop departmental annual Plans, Recruitment & Training plans for
contiuous improvement.
4. Develop and manage departmental budget, analyze variance and identify cost
reduction opportunities.
5. Facilitate and chair monthly meetings to consider the current status of H&S activities,
address critical areas for improvement and to resolve problems.
6. Conducts scheduled/unscheduled meetings, when required, with HSE department Staff,
site team or client's Management to address issues.
7. Conduct Operational & Management Review meetings at planned intervals to present
plans related to development, enhancement or implementation of the System.
8. Continually look for the ways to improve methods.
9. Recommend QHS Man of the Month, Man of the Year, Spot Award, Near Miss and Best
Suggestion Awards nominations for Management's approval.
10. Provide leadership & direction for departmental HSE activities.
11. Ensure Company's H&S reputation throughout and compliance to OH&S Management
system.
12. Develop and conduct effective corporate wide awareness programs.
13. Approve OH&S Internal Audit Plan.
14. Monitor site(s), Internal or External Audits results, raised Non conformances
report (NCR).

4.2 HSE Manager (At Head Office)


The HSE Manager will be stationed at Head office and he will be bearing the overall
responsibility of Formulating, Periodical reviewing, proposing strategically changes in policy,
reporting to the management etc., of the Safety and Loss prevention program of the company.
His main responsibilities are to:
1. Ensure that OHSMS, H&S Policy and objectives are communicated.
2. Ensure and establish best H&S practices and continually seek ways to improve standards.
3. Communicate and maintain all legal, statutory and other requirements and ensure its
implementation.
4. Ensure effective implementation of Emergency Response Plans at corporate & project
level.
5. Conduct Health & Safety meetings and review the achievements of objectives and
improvement in OH&S Management System.
6. Ensure that all personnel have received the appropriate Health & Safety
trainings.
7. Investigate and report all recordable accidents events to management and concerned
authorities.
8. Maintain safety records and certificates for future correspondences.
9. Conduct site visits to ensure that all Health, Safety, Environment and loss
prevention measures are strictly followed.
10. Plan and implement HSE programs and promotional activities and periodic reviews
for improvement.
11. Establish and maintain incidents accident reporting system, investigation
and analysis as per OHSMS procedure.
12. Advice Manger TQM on all matters pertaining to HSE including compliance to
Kingdom’s legal and statutory requirements.
13. Instruct sub-contractors on OHSMS procedures and ensure that their employees are
advised to comply it.
14. Ensure to collect site monthly and weekly H&S data for analysis, management
information and necessary actions.

4.3 Project Manager


The overall responsibility in implementation of Safety & Loss Prevention program (S&LPP)
during the execution of this project lies on his shoulders.

His main responsibilities are to:


1. Ensure implementation of Health & Safety policy and OH&S Management System.
2. Ensure compliance to HSE, legal, contractual & other requirements at site.
3. Recommend the employee(s) for Health & Safety training(s).
4. Direct resources for effective implementation of OH&S System.
5. Handle and execute Customer Complaints I Nonconformance(s) raised.

6. Negotiate and resolve issues related to Health & Safety where it has impact
on other activities, systems and project performance.
7. Monitor and manage project H&S budget, risks and operational issues.
8. Ensure project meets all HSE requirements and objectives.
9. Chair weekly meetings and discuss issues related to Health & Safety.
Advice required Corrective and Preventive Actions.
10. Communicate project H&S status to management and other
relevant stakeholders and/or involved parties.
11. Identify hazards and allocate resources for appropriate controls.
12. Authorize and support HSE representative to stop work when there are
failures to execute work under safe conditions or denying OHSMS requirements.
13. Ensure that all employees are wearing PPEs.
14. Ensure that H&S related project documentation are maintained.

4.4 Site Manager


Site Manager is responsible to:
1. Ensure the implementation of OH&S Management System.
2. Comply with all OH&S rules and regulations.
3. Encourage employee's motivation programs.
4. Conduct regular site walks thorough reviews.
5. Liaising with clients and reporting progress.
6. Supervise incident I accident investigations and its reporting to concerned.
7. Arrange trainings for new inductions.
8. Ensure validity of all cold and hot work permits.
9. Regular monitoring and checking of all OHSMS procedures.
10. Coordinate monthly safety meetings with interested parties.
11. Participate in Emergency Response plan drills.
12. Communicate emergency I unpleasant events to management.
13. Ensure availability of required PPEs.
4.5 Site Engineer(s)
After Site Manager, Site Engineer(s) is/are responsible for the implementation of Safety
& Loss prevention program during construction at site.
The main responsibilities are:
1. Assist in the implementation of OH&S Management System.
2. Ensure that all HSE and project based loss prevention programs are implemented
and working efficiently.
3. Coordinate OH&S related matters with client and other interested parties.
4. Reporting to SM & PM about incidents and accidents.

5. Assist PM & SM in incidents and accidents investigations.


6. Review reports to ensure the effectiveness of corrective and preventive actions taken.
7. Participate in Emergency Response plan drills.

4.6 Testing & Commissioning Engineer

Testing & Commissioning Engineer is the key person in implementation of S & LPP
during testing activities. He is responsible to take care for all work related safety
requirements at job site, where testing and commissioning work is going on, in
order to avoid any unwanted risk event.

His key responsibilities are:-


1. Ensure safe working conditions.
2. Ensure safe working of equipment’s and staff according to standards specification.
3. Establish and follow safe testing procedure.
4. Investigate problems, diagnose it and suggest trouble shooting.
5. Supervise team of Technical Staff.
6. Ensure all safety rules and regulations are followed at site.
7. Liaising with project team.
8. Interacting with client & consultant on planned intervals regarding testing and
Commissioning activities.

4.7 HSE Supervisor/Officer


Safety Supervisor/Officer at site reports directly to Project Manager operationally and to HSE
Manager H.O administratively. He is responsible for the establishment of an effective safety
culture which is based upon the company Safety Policy, meets the company's contractual
responsibilities.
His principal responsibility encompasses minimum the following functions:

1. Implement O H & SManagement system and loss prevention program at site.


2. Assist for the assessment of possible hazard & risk involved.
3. Ensure availability and usage of required PPe’s during work.
4. Ensure regular inspections of work area, tools & construction equipments
before start of work.
5. Ensure, before start of work,the validity of tools used at site.
6. Maintain regularly inspection checklists as per OHSMS requirement.
7. Ensure inspection of First Aid items on regular basis.
8. Ensure safety of all testing and commissioning equipments.
9. Ensures installation of adequate fire fighting equipments both at site and
offices.
10. Attends the OH&S meetings regularly.
11. Conduct weekly Safety talks and Toolbox meeting(s).
12. Ensure H&S orientations for all newly arrived workers at site before sending them on

their assignments.
13. Ensure posting of all warning signs at site and area barricaded.
14. Carry out daily inspections to see that safe working methods are in operation.
15. Prepare, maintain and communicate records of all injuries and property damages.
16. Promote motivation activities to increase Health, Safety and Environmental awareness at
site.
17. Stops work and immediately inform Site Manager & Safety supervisor in case of
emergency.

4.8 FIRST AIDER

1. Provide First Aid to injured or ill employees at site.


2. Carry and accompany the causality for further medical treatment to the nearest hospital {if
required).
3. Ensures the complete recovery and fitness of injured or ill employee before joining duty.
4. Applies artificial respiration I administers oxygen in case of suffocation or breathing
problem.
5. Administer medication.
6. Assist Safety Supervisor, Officer, Engineer in implementing Health & Safety
system at site.
7. Keep records of all treatment and first aid cases

4.9 FOREMAN

1. Assist Safety Supervisor and Officer in the implementation of OHSMS.


2. Ensure Safety of equipment{s) under use.
3. Check that all workers at the site have proper tools.
4. Maintain good housekeeping.
5. Participate in toolbox meeting with his crew.
6. Dealing all the workers so that they may perform their work smoothly.
7. Ensure safety of Crane and Equipment operators while operating heavy equipments.
8. Follow Loss prevention program and company OH&S system and have a close look over
the safety precautions, concerned with tools, equipments and most importantly with
workers.
9. Ensure provision of adequate Personal Protective Equipment {PPEs) to all of his team
members before start of work.
10. Ensure that all new employees have gone through safety induction and training program.
11. Make best communication with the Supervisors and Engineers.
12. Report incident, accident and risks to site Management.
13. Assist site management in incident and accident investigation and analysis.
14. Assigning proper job to an individual at site.

4.10. Technicians and Workers


1. Ensure correct tools and equipments for the job.
2. Follow Loss Prevention Program while working.
3. Provide technical assistance and resolution in case of engineering or technical problems.
4. Keep tools and equipments in proper working condition.
5. Maintains work area clean all the times.
6. Refrain from horseplay and abuse of safety devices, equipments, and welfare facilities.
7. Check and guides whether the co-workers understand the work HSE
requirements or not.
8. Follow instructions of seniors safety representative in case of emergency condition.
9. Communicate report potential hazards, if any, to site management HSE team.
10. Give suggestions of improvement about Health, Safety & Environment.

4.11. Sub-Contractor and their workforce


Either Sub-Contractor shall follow the IHCC's Health,Safety & Environment rules and
regulations stated in this program exactly or they will submit their own program
which shall be evaluated and approved by the IHCC before putting them into work.
They are mainly responsible to:
1. Shows presence at worksite when Subcontractor is performing any task(s).
2. Attend safety meetings as required.
3. Ensure daily and weekly inspections and communicate it to site management of IHCC
4. Assist in risk analysis when required.
5. Submit all Health & Safety incidents I accidents reports to IHCC PM and HSE Manager.
6. Ensure good housekeeping at their work sites, offices & residential areas.
7. Ensure submission of Environmental Plan if required.

Tasks & Responsibilities will be practiced in accordance with IHCC OH&S Management System
Procedure SP4.4.1.

5. RULES AND REGULATIONS FOR PERSONNEL AND EQUIPMENT SAFETY

1. As an agreed principle and guideline, IHCC Construction Co will follow all the
general safety rules promulgated by SEC for its employees laid down in their
"ACCIDENT PREVENTION MANUAL."

2. Before start of the work, all involved workers will be made aware to:
a) Complete understanding of the work to be performed.
b) Properly planned procedure of the work
c) Possible hazards and danger points

d) Specific job assignment of each worker


e) Safety aspects as a continuing part of day-to-day work

3. Before start of the work, every personnel present at the work site will make sure that:
i) Working conditions are safe and sound.
ii) No unsafe situation is expected to creep in.
iii) Prior arrangement for any unforeseen hazardous situation exists.
iv) First aid medication is available and accessible.

4. Every worker will take care of his personal safety in the first instance. Safety of his co-workers
and of the equipment will be given equal importance.
5. Work permit will be obtained from the authorized issuer by any of authorized receiver.
6. All working staff engaged near live overhead lines or underground cables will wear
safety shoes, leather gloves, helmets and safety goggles. At areas of high noise levels,
workers will use ear muffs.
7. All the existing electrical circuits will be taken for granted as live circuits
unless otherwise confirmed.
8. All de-energized electrical circuits will be properly grounded and tagged.
9. Adequate illumination will be made available while working at night.
10. Adequate signs, warning logos, identification marks will be used at work sites
during the progress of work.
11. As far as possible, public access to work sites shall not be allowed.
12. Separate parking lot will be made for all vehicles. Extra care will be taken for
vehicles entering the work site to avoid contact with any live part.
13. Wherever materials are stored, minimum 1.5 m space will be kept to avoid spread of
fire and for inspection purpose.
14. In no case, smoking will be allowed at the work sites.

6. GENERAL SAFETY PRACTICES

IHCC is OHSAS 18001:2007 Certified Company and works according to applicable National
& International Health & Safety Standards.

a) Site Planning
Site layout is attached in attachment E.

b) Personal Protective Equipment


Provide and require all workers to use the necessary safety and personal protective
equipment required by the Client such as helmets or hard hats, goggles, gloves, face
shields, safety belts, safety shoes, breathing apparatus (if needed) etc.

c) Housekeeping
Housekeeping is a most important factor in safety and fire prevention.
A program of good housekeeping will be established at the beginning of the job and will
be enforced as the work progresses. Following are the main feature of housekeeping:

Adequate number of workers will be deputed for the housekeeping as and when
required.
Trash container will be provided on each job locations. Garbage will be removed from the
site on daily basis.
All unserviceable leftover material will be removed from the work site on daily basis. All
access will be kept free of hazards.
No electrical supply leads or cable will be allowed in unsafe manners.
No any kind of piles/ heaps will be allowed to remain on work site.
The layout of material storage area, site offices, parking areas, stores, etc., is available
in Temporary Site Layout Plan (Attachment-E) to provide safe access into the site by
pedestrians, vehicles, and equipment.

d) Hand and Electrical Portable Tools


All the hand electrical tools must be an approved type and shall be properly
grounded. All connection shall be electrically and mechanically sound and properly
insulated. Moving parts of power tools, except those parts which must remain exposed
for the efficient operation of the tools shall be securely guarded.

Hand and power tools shall be used only by men who have been trained and
instructed in their use and will be checked and properly maintained regularly.

e) Ladders
Ladders of appropriate length shall be used.
Ladders with metal reinforced side rails & those ones that are wet shall not be used near
electrical equipment with exposed live conductors.
All ladders shall be checked every three months for any damages, deterioration, wear &
tear, corrosion & structural failure.
Both side rails shall have secure footings. The area at the base of ladder shall be kept clear.
Ladders shall not be positioned against insecure structures such as boxes, drums, or weak
walls.
The user shall always face the ladder while ascending or descending.
The user shall not carry any materials or tools in his hands while ascending or
descending.
Only ladders shall be used for climbing.

f) Scaffolding
A scaffold is an elevated working platform for supporting both men & materials.
• Scaffolds shall be adequately braced and supported cross bracing must be
securely fixed.
• Scaffolds of more than 4 meters high shall be securely fixed to the structure.
This can be changed due to the location and size of scaffold column.

• "Standards" or upright shall be plumbed to check that they are straight.


• Firm footings shall be provided for each "Standard" by means of metal plates.
• A Minimum of Scm (2 inch) diameter tubing shall be used for scaffolds having
height up to 23 meters (75 feet). Other scaffolds shall be as per manufacturer
recommendation.
• Guard rails shall be secured to standards by firm connections.
• Bracing shall be provided in all types of scaffolds to add rigidity.
• Working platforms shall be free from grease, oil or slippery material.
• Changes in design & construction of a scaffold shall not be made without
engineering approval.
• Scaffolds shall be checked for any loose connection, damage prior to use.
• "Standards" shall be pitched in the base plates. The size of the plates will be
considered as per job site but should not be less than 15x15 em (in any case).
• Approved and good condition couplers shall be used.
• "Ledgers" shall be secured end to end by means of sleeve couplers/ connectors.
• "Transoms" is a structural part of scaffold and shall be secured to standard with
Go-load bearing couplers
• "Braces" shall be fixed to "Ledgers" with goo load bearing couplers. If this is
impracticable, a swivel coupler is used.
• All scaffolds except certain tower & mobile scaffolds shall be tied to firm
structures to prevent accidental movement.
• "Ties" shall be in position at least every 4 meters vertically and 6 meters
horizontally. "Tie" assembly connections shall be made with a go-load bearing
couplers.
• "Tags" will be used for scaffolding by the HSE Officer which clearly identify
that the scaffolding are safe for use or not.

g) Lifting and Rigging


Items of equipment used for lifting and rigging shall be in good condition
and suitable for the work. Crane shall be inspected and approved by
Client crane inspector and current inspection sticker will be displayed on the
windshield.
A crane operator shall possess a valid 3rd Party license and have a
sufficient experience for the machine he is operating. Crane shall be fitted
with a means of indicating to the operator the maximum safe workable load.

Crane shall be operated only on a firm level standing. The Crane


Operator shall act on signals of one man only. The signal man shall be
easily recognized and shall use the international standard hand signals.

PART – III

7. WORK PERMITS
1. Work permit is an authorization to do or perform certain work on an energized
equipment or part of the energized system in a restricted area. It is issued by an
authorized employee of the concerned proponent organization. The receiver will also be
an authorized person, who will be delegated the authority of receiving the work permit by
SEC Before issuance of the work permit both the issuer and receiver will ensure the safe
working conditions of personnel and equipment.
2. After issuance and receipt of work permit, the equipment or the circuit involved will be
switched-off by the on duty switchman of the proponent organization. Appropriate tags will
be placed on equipment under work permit as long as the permit is valid

3. After completion of the work, the work permit will be closed-out by the authorized issuer and
receiver after observing all closing-out requirements. The tag will be then removed by the
authorized person.

4. Every care will be taken while restoring the electric supply. No job is more urgent or no
service is more important than the safety of the workers. Therefore, safety of personnel must
be ensured before energizing the involved electrical equipment or system.

5. In case the receiver of the work permit needs to leave the work site (for genuine reason), he
will inform the issuer and designate other authorized receiver of work permit to look after the
work in his behalf.
6. The work permit will be displayed at the job site.
7. Receiver of the work permit will stay in the work place for the complete duration of work
permit
8. Issuer/receiver of the work permit shall stop the work if the facility is found unsafe.
9. Work permit shall be filed for a minimum period of three months.

8. SITE PLANNING,HOUSEKEEPING AND SITE ACTIVITIES

Prior to the start of any contract, the contractor will ensure that construction will be conducted
in a safe manner and an effective planning will be done to eliminate or reduce accidents during
construction. The factors which will be considered during initial planning stage are: Hazard
Identification Plan,loss prevention program, protection of employees and equipment,
transportation,lifting equipment, excavation, scaffolding and work in restricted areas requiring
work permits.

The contractor will implement construction safety execution plan to minimize disruption to
construction activities,identify project safety concerns, avoid hazardous situations and provide a
single source of procedural information for use during construction activities. For this purpose it
will be made sure that specific meetings, audits and inspections, training programs, promotional
activities and record keeping as well as day to day consultation on
Safe Working Practices are encouraged and formalized.

Contractor will follow client standards for required housekeeping on site and all personnel will
be communicated and informed. Trash, debris and refuse shall be collected daily. All employees
shall clean their respective work areas daily before quitting. Material shall be stacked
appropriately and also walkways & Aisle shall be kept clean. For controlling the dust, water
tanker car shall be used.

Prior to commencement of construction activities the Project Manager shall ensure the following
initial arrangements for Project start-up are put in place;
 Run and introduce awareness program among employees
 Requirements for fire and safety equipment are available.
 Safety orientations for senior staff and the workforce are carried out. Establish proper
communication channels
 Training requirements are established.
 Ensure suitable management arrangements are in place
 An interface with local health officials and emergency care facilities are established.
Liaison procedures with client Emergency services are established.
 Ensure adequate welfare facilities on site.
 Establish a comprehensive record keeping system including all relevant report forms and
procedures.
 Prepare safety incentive schemes.
 Prepare safety instructions, emergency procedures and associated documentation.
 Prepare safety instructions, emergency procedures and associated documentation. Establish
procedures for heavy equipment, crane, rigger and work permit certification.
 Hold regular tool box meetings

9. CONSTRUCTION SAFETY PLAN


A) EXCAVATION WORKS
1. Before starting excavation work, it will be ensured that underground facilities like
water pipes, gas distributions or electricity cables are non-existent. In case of their
existence, clearance from the concerned agencies or companies will be obtained.
2. The workers involved in excavation will wear warning vests, whenever necessary.
3. The soil being excavated shall not be left at the edge of the hole/trench. If possible
retaining barrier will also be used.
4. All excavations will be guarded or barricaded throughout. In order to warn the

mobile equipments being used in the work site warning signs should be visibly
placed during day time and proper lighting arrangement (covering all excavated area)
be available after sunset.
5. Extra care will be exercised if underground cables are involved in the excavation work.
The cables have to be properly identified by the department concerned. The energized
cables will be de-energized and grounded.
6. If an excavation is deeper than 1.5 meters, adequate bracing and shoring shall be provided
or
the trench shall be sloped.
7. Mechanical excavators shall not be used within 3 meters of known under ground utilities
and obstruction to avoid accidents due to possibility of damage.
8. Shoring shall be used immediately or sides shall be sloped to a safe angle to avoid
collapsing
of sides of an excavation.
9. Wooden planks or temporary walkways shall be installed over an excavated area for
use by workmen crossing to the other side and suitable ladders are available to make access
easy to and from excavation.
10. In case of any damages to the underground facilities/utilities the respective company
shall be notified immediately, and the same effected area shall be barricaded till the
condition is under control.
In case of any damage to the gas or oil pipeline:
i Warn the residence of the area
ii. Inform local fire station
iii. Inform local security department

B) Backfilling
Backfilling, a part of construction process to protect foundations and removal of trench
supports shall be accomplished first by backfilling up to a level allowing for the removal of
the lower braces. Another layer of backfill shall be positioned in the trench to the next layer
of braces to be removed. Removal of trench supports shall progress together with the
backfill from the bottom of the trench.

In unstable soil, ropes shall be used to pull out the jacks or braces from above after
employees have cleared the trench. All excavations shall be backfilled and consolidated,
and the surface shall be left in good condition as soon as is practicable.

C) STEEL STRUCTURE ERECTION


1. While erecting steel equipment it will be ensured that all cables, chains, ropes,
blocks, slings and gin pole connections are correctly sized with proper strength.
2. Cranes, mechanical and electrical equipment shall be operated by experienced
operators only.
3. All steel members will be checked that they are firmly bolted with each other.

4. The workers involved in steel erection process will wear safety shoes, hard hats,
leather gloves and other required Personal Protective Equipment.
5. No person or vehicle will be allowed to pass under crane or through working area
while it is being erected, erection site must be restricted through
necessary barricades.
6. Rigging equipment will be inspected prior to use. Any chain having signs of rusting or
wear and tear will be replaced. The equipment will not be loaded in excess of its safe
working load.

D) WELDING & CUTIING


1. Whenever welding, cutting or brazing is required in a restricted area, a valid work
permit will be obtained from the authorized personnel
2. Welding personnel will use the protective equipment and clothing which include
welder's gloves, sleeves, hand shields, helmets, goggles etc. Outer clothes will be
free from oil and grease.
3. Eye protection will be applied while chipping welds.
4. Welding screens and shields will be used in all cutting and welding process.
5. No inflammable liquid will be placed in the vicinity of the area where welding
process is going on.
6. Adequate ventilation will be ensured during welding. However, in no case pure
oxygen will be used for this purpose.
7. Gas cylinders used for cutting will be transported, stored and used with secured caps
and in the upright position. Carts or trolleys will be used to shift the cylinders.
8. The fuel gas and oxygen hose will have distinct colors or identification marks so that
they cannot be interchanged.
9. Welding torch tips used for cutting or welding will be cleaned with some proper
cleaning device. Any torch formed bearing leaks etc. will be replaced.
10. Torches will be lighted by spark lighters.
11. Oxygen and gas cylinder pressure regulators and gauges will be checked for their
right working before use.

12. In case of electric welding, the welding cables and other current carrying parts will
be fully insulated.
13. All ground connections must be tight, dry and clean.
14. Welding slag shall not be allowed to fall on top of gas cylinders flammable materials
or gas pipe line.
15. The cylinders shall be secured with a retaining chain to prevent them from falling
down; they shall not be stored near elevator, walk-ways, and stairways.
16. Portable dry chemical fire extinguishers shall be located nearby welding and cutting
areas {Fabrication Workshop).
17. An oven will be provided for the electrodes being used in heavy welding work.

E) Cartridge Power Tools


Cartridge operated tools require strict control on their issuance and usage. A register of
withdrawal and I or uses will be maintained at the Project Stores and an inventory and
inspection of cartridges will be performed daily by the store keeper to ensure accountability.
Craftsmen authorized by their Site Engineer shall be allowed to withdraw and use the
particular tool in the Project operations. Used cartridges shall be retrieved and returned to the
stores for control and disposal purposes. Users of cartridge operated tools shall carry on their
person a certificate from the tool manufacturer (or its agent) stating the user has completed the
necessary Safety Training on the correct operation of the particular tool.

F) Working at Heights:
When work cannot be carried out from grade level, a safe working scaffold and safe means of
getting to and from the work place will be provided. IHCCwill ensure that standard or mobile
scaffold will be erected for craftsmen working six (6) feet above the grade level. For fall
protection, the use of full body harness shall be mandatory at work areas six (6) feet or above
the ground which do not have approved scaffolds, barriers or other protective measures.
In critical areas where scaffolding cannot be erected, the use of a man lift basket may be
adopted. All harnesses are inspected by the users before each use and by their Supervisor each
day. Lifeline systems where installed are inspected daily by IHCCdesignated competent
person and the HSE Supervisor/Officer.
A Competent Person is responsible for the supervision and inspection of scaffolding.
Scaffolding is inspected daily. A tagging system is in use to indicate the status of scaffold
availability. HSE Supervisor/Officer administers the tagging system.

H) Concrete Work
During concrete work, contractor shall consider following precautions
1. Proper clothing {coverall with long sleeve and full length pants), Alkali resistance gloves,
water proof boot, eye protection shall be worn.
2. Proper working platform with guard rails or fall arrest system shall be provided.
3. Concrete truck outrigger pads shall be deployed on the proper stable support and truck
wheel shall be blocked and brakes set when discharging on slope.
4. Concrete pump operator shall use proper personnel protective equipment.
5. Effective Voice and signals messaging shall be done between the concrete pouring crew.
6. Safe access condition shall be ensured.

WORK SITE
The Work at "Work Site" shall be carried out as per approved safety procedure.

• Work site shall be covered with the fencing from each side.
• An entrance for the transportation of material will be left on the existing roadside.

• The gate shall have 02 panels approx. size will be 2.5x2 meters of each panel.
• A barrier shall be installed inside the gate with the "STOP" mark.
• Safety and Security orders and signs will be place on the gate.
• A safety and security office will be provided after barrier beside the fence.
• A security guard will be deputed on the main gate of site for 24 hours.
• Assigned security guard will be an educated, physically fit and uniformed.
• Record keeping and gate passes will be systemized in the safety and security office.
• Traffic and safety sign, fire orders will be placed on the required places.
• Smoking will be strictly prohibited in the work area.
• A proper waiting room will be provided in the safety and security office.
• PPEs for visitor will be arranged at security and safety office.
• No one will be allowed to visit the site without PPEs, identity cards.
• Company's monogram/logo will be pasted on all equipment and vehicles.
• SOOW light shall be posted all around the site at the distance of 25 meters maximum.
• Large size of window will be left in the safety/security office.
• Fire alarm switch will be fixed in the safety/security office.
• Telephone service will also be provided in safety and security office.
• A first aid room will be provided in central location of the work site.

Part IV
11. FIRE PREVENTION & FIRE FIGHTING

A) FIRE PREVENTION
To avoid the risk of fire and to minimize the losses due to fire a well organized fire
prevention program will be established at site on each stages to achieve the objective that:
i. Fire may not occur.
ii. If occurs, may not be spread.
iii. If spread, the losses shall be minimum

The fire prevention program will be contained as under:


1. Fire prevention awareness (through weekly toolbox meeting) discussing the topics
i. Classification of fire.
ii. Classification of construction.
iii. Nature of fire.
iv. Material storage.
v. Daily inspections.
vi. Hot works
vii. Housekeeping and other subject related to fire prevention.

2. No exposed wiring and lose connection will be allowed; only conduit wiring
will be permitted.
3. Distribution boards and other supply will be inspected regularly.
4. Smoking will not be permitted in storage areas.
5. Gas heater/ burner will not be permitted in cabins.
6. No chemicals and liquids will be permitted in the warehouse.
7. Sites will be kept clean and free of highly inflammable/combustible liquids or
materials, stray wooden logs, waste papers, cartons, rubbish, trash, etc. for fire
prevention.
8. Inflammable liquids and combustible materials whenever used shall be handled and
stored with extra care. No sparking or ignition should take place in the nearby area
these are being used. Furthermore, this area will be well ventilated and will be cleaned
from any debris or rubbish etc. Smoking will be strictly prohibited.
9. Stacking, storing and piling of inflammable and combustible materials
will be handled with utmost care. Following are minimum spacing between
hazardous units and other facilities.

Ordinarily flammable (low to medium pressure) 7-23m


Highly flammable (high pressure) 30-46m

10. Office equipment Electric apparatus including AIC units, fans & heaters, shall be
switched off before leaving the work sites.

11. During unloading of fuel, trucks engines must be stopped & trucks & other
unloading equipment should be well earthed to prevent static electric charge.
12. The electrical circuits, wiring, conduits, accessories, fuse boxes etc. will be kept in well-
maintained and orderly conditions. These will be free from cracks, leaks, loose joints,
heating effects, worn-out insulators, faulty sockets, broken pins or plugs etc.

B) FIRE FIGHTING
1. Wall-mounted fire extinguishers will be installed at visible, accessible and convenient
places. Clearance between top of the fire extinguisher and finished floor level will be 3
feet (1.07 meters) maximum.

2. Water tanker will be arranged I provided at work site.

3. Fire extinguisher shall be inspected regularly for charging pressure.


Workmen shall be trained to use fire extinguisher. Instructions about how to
use fire extinguisher in case of fire shall be posted on each and every fire

extinguisher.

4. Vehicles like trucks, trailers, cranes etc. will be equipped with fire extinguishers.

5. Combustible and incombustible materials will be stored separately and properly labeled.

6. Transfer or mixing of flammable liquid shall be done in well-ventilated area.

7. Flammable and combustible liquid such as paints, oil shall not be stored in open
containers and shall not be stored near a heat source.

8. Emergency action plan will be prepared at the 1st stage of the work execution.

9. Evacuation plan will be also prepared and practiced as per site requirement.

10. Fire Extinguishers shall be provided closed I at appropriate location of the parking area.

11. Important points of the site will be displayed with the contact numbers of nearest located
firefighting, fire brigade department for rapid flow of information.

12. PERSONAL PROTECTION


Personal protective equipment, periodic environmental monitoring and bio-surveillance will be
used to help to protect all employees against exposure to safety and health hazards which
cannot be eliminated.
Contractor shall provide personal protective equipment i.e. hard hats, safety glasses, gloves,
protective clothing and safety boots, and shall be used and maintained in a sanitary and reliable
condition.
Other PPE"s such as hearing and respiratory protection will also be provided to prevent
illnesses and unwanted health effects. HSE Officer will check and inspect and make sure that
everybody working at project site is using PPE's so that the risk of hazard may be reduced.
Similarly contractor shall provide harness/ line yard/ lifelines to work at elevated area and
will be regularly checked and monitored

13. MATERIAL HANDLING & STORAGE


A. Manual handling of materials:

Safe lifting can be carried out by correctly using leg & thigh muscles and positioning the body
properly. Leg and thigh muscles are stronger than back and arm muscles and can bear more
physical effort I strength. If a man bends his body the spine is arched & become weak.
If the spine is kept in a straight position while lifting, the effort is concentrated on the
stronger leg & thigh and arm muscles allowing the load to be lifted easily & safely.
The workmen shall be trained for lifting loads manually with demonstration during there
training sessions.
a. Any weight shall be lifted by the capacity of the lifter's physical condition.

b. The proposed route of travel shall be clear.


c. Secure grip shall be ensured.
d. If required lift it twice.

Proper attention should be paid while handling material. Ascertain whether it can be
done manually or it needs mechanical equipment

b. Remove any sharp objects present.
c. Wear appropriate protective equipment.
d. Decide where to place the object.
e. Survey the area around the object and proposed route of travel. Remove any obstruction
present.
f. Get sufficient help to lift and transport safely.
g. Lift within your physical capacity. Keep feet close to object. Crouch close to the load
with knees bent. Use legs and arm muscles.
h. While two or more persons are transporting one extra man should give the signals while
others march with the load.
