Awol Notice

An HR notice informs an employee that they have been absent without leave for over [number of] days/weeks between [dates]. HR records show no leave form was filed for this period. Calls and texts to the employee's cellphone went unanswered. The employee must explain within 5 days why disciplinary action should not be taken for being absent without filing leave and for abandonment of work.

  • Notice of Absence Without Leave: This is an HR notification letter informing the recipient of their prolonged absence without leave, requesting their compliance and response.

Subject: Notice to Explain Absence With-Out Leave

Our records reveal that you have been absent for more than _______days/weeks without proper
notification covering the period ________ to this date, ____________ It does not appear either that you have
previously filed any leave form for the period.

HR personnel also called your cellphone number: ____________ but no one answered. We also sent
you text messages on your cellphone asking you to give us your condition but we never received any reply.

In this regard, please explain within five (5) calendar days from receipt of this notice why no
disciplinary action should be imposed upon you for being absent without filing the appropriate leave form.
Likewise, explain further why no disciplinary action should be imposed upon you for abandonment of
work.
