AFFIDAVIT OF INDIGENCY

I, MARIA THELMA P. CAINTOY, of legal age, Filipino, and a resident


of Brgy. Pagsang-an, Abuyog, Leyte, after having been sword in accordance
with law, hereby depose and say that:

1. I belong to the lower strata in life, having no sufficient source of income;

2. My gross income and that of my immediate family do not exceed an


amount double the monthly minimum wage of an employee nor do I
own real property with a fair market value as stated in the current tax
declaration of more than Three Hundred Thousand (P300,000.00)
Pesos;

3. Attached hereto is a Certification of Indigency issued by the Office of


the Municipal Social Welfare and Development (MSWD) as Annex –
“A”, as well as an Affidavit of Two Disinterested Persons to support the
truth of the foregoing affidavit, as Annex – “B”;

4. I am executing this affidavit to attest to the truth of the foregoing and


for purposes of entitling me to avail of the free legal services of the
Integrated Bar of the Philippines.
