The goal is for the patient to collaborate with healthcare providers and explore management options for hypertension within 3-5 days. Interventions include home visits, education on hypertension, and assisting the family to identify management strategies. The evaluation plan aims to assess the patient's understanding of hypertension and ability to identify management approaches within 2-3 hours and 3-5 days.

LEARNING INSIGHT:
As a student nurse who is exposed in the community, I was able to appreciate the Madeleine Leininger's Culture Care Theory, in relation to my patient’s case where
in her culture has something to do with the fate of her life that sometimes hinders her way to better health condition, as a health care provider you also have to consider it
and respect it as it is. However, since it already hinders her condition I have to reiterate and help the client change her decision as much as possible. Moreover, I was able
to learn that your role as a nurse does not only applies in the hospitals but it also requires the same in the community especially because you are dealing with a large
group of people who needs exactly the same care as the patients in the hospitals
