It is to be completed and signed by a ‘responsible’ person familiar with the vessel,
usually a representative of vessels Technical Manager as indicated in Q2 of this
questionnaire.
Submission of completed and signed questionnaire and copies of requested documents
is essential prior to a formal decision on the vessel.
