AFFIDAVIT OF DISCREPANCY

I, ___________________________, Filipino, of legal age, and presently residing at 77 Cavite


St., Bonifacio Subdivision Tandang Sora, Quezon City, after having been duly sworn in accordance with
law hereby depose and state that:

1. I am presently the _________________, registered with Department of Trade and Industry


(DTI) and engaged in the female garments business;

2. Since childhood, I have always used the name _________________ which is the name
reflected on my baptismal certificate, attached hereof;

3. When I secured a copy of my Certificate of Live Birth from the Local Civil Registrar of
Manila and from the National Statistic Office (NSO) sometime in the year 2000, I was
surprised to discover that my name recorded on said Certificates of Live Birth was
___________________, child of ________________________ and
________________________. Copies of the Certificates of Live Birth from the Local Civil
Registrar of Manila and from the NSO are attached respectively;

4. In all my school documents and employment records, I have always used the name
________________________as shown by copies of my diploma from the University of Santo
Tomas (UST) when I graduated with the degree of Bachelor of Science in Commerce;
diploma from UST when I graduated with the degree of Associate in Secretarial Science;

5. I got married to _________________ on ________________ based on attached Marriage


Contract hereof. From that time, I have been using my married name as

6. The person registered as ____________________ in the Certificate of Live Birth issued by


the Local Civil Registrar of Manila and from NSO and the one recorded as
