That I, WEVELYN HORTALEZA RIMANDO of legal age, Filipino citizen, married, and a
resident of Brgy. Sevilla, City of San Fernando, La Union do to hereby NAME, CONSTITUTE AND
APPOINT ALICE O. LEDAMA, also of legal age, single, and a resident of #275 Brgy. Carlatan, City of
San Fernando , La Union, to be my true and lawful attorney-in-fact, for us and in our name, place and
stead, and for my own use and benefit, to do and perform all or any of the following acts and things,
namely:

1. To represent me before the DTI, City of San Fernando, BIR in connection with the
processing of my business closure under the business name I’ Scream Food Truck
located at Brgy. Carlatan, City of San Fernando, La Union;
2. To pay any sum or amount requisite for the closure of the business;
3. To receive all necessary documents requisite for the closure of the business;
4. And to generally do and perform all and every acts which may be requisite, necessary or
proper to carry out the above purposes.
HEREBY GIVING AND GRANTING unto said attorney-in-fact full power and authority to do
and perform every act and thing whatsoever requisite and necessary to be done in and about the premises,
as fully to all intents and purposes as I might or could do if personally present, hereby ratifying and
