Sir:

During summer vacation, teachers are sometimes required to come to school for the best
interest of the service. You were designated as property custodian and on this positon you
are required to report to school even vacation as stated in your job description.
State your reasons why you will not be charged administratively for the following:
1. non attendance in our school activities such as
a. faculty meetings ( From May 25, 2018 to June 1, 2018 meetings)
b. Brigada Eskwela 2018 ( May 28- June 2, 2018) , especially the Division Brigada
Eskwela 2018 Kick off Ceremony which was held in our school

2. No replies from texts and calls sent.
3. Amidst the home visit done, urgent report were not submitted on time s

This office requires you to submit your explanation letter within three (3) days upon receipt
hereof.

Sir:
Owing to your concurrent position as Property Custodian of the district you are required to report to
your official station to discharge your function. As a teacher you are also required to report if such attendance
serves the exigency of the service.

Under the School Calendar of Activities for SY 2018-2019 , teachers are earnestly required to report,
but to my recollection and records show you failed to attend such, despite diligent act to sufficiently inform
you on the said activities via calls, text messages and the likes, to wit;

1. Faculty Meeting (From May 25 to June 1, 2018)


2. Brigada Eskwela Kick Off Ceremony
3. First PTA General Assembly

More so, home visit was done for the past to inform you on required report which long and overdue but
still report where not submitted on time.

In view of the above, explain within 72 hrs why you are not be charged administratively for
insubordination and gross negligence of duty both punishable under civil service law with heavy penalties.
