Preparation of the performance evaluation is an interactive process where there is an
exchange of information between you and your supervisor. Please provide your supervisor
with information related to your performance in terms of expectations being fulfilled as per
governance set up memo.

As a pre-elementary teacher I always do my best to follow all the rules and regulations of the
school.

Part II. Goals:


Provide an assessment of your success in reaching the goals set/targets(6acts) at the
beginning of the last evaluation period.

I always ensure that I follow the 6 acts of the school provided that I act as a role model to my
students.
