AFFIDAVIT-COMPLAINT

I, CLARO T. PATAUEG, of legal age, Filipino, and a resident of


____________________________, accuse FE MALSI of falsification committed as
follows.
1. On November 6, 2018, Fe Malsi submitted an application letter,
attached as Annex “A”, on my behalf with my falsified signature signifying my
alleged intent to monetize my leave credits. It was stated in this letter that I
needed the money to purchase medicines because I was suffering from diabetes
and hypertension.
2. There was also an authorization letter dated November 8, 2018 with
my forged signature, attached as Annex “B”, alleging my intent to authorize Fe
Malsi to claim the monetization of my PAFEMCO loan on my behalf.
3. I had come to know such facts on January 4, 2019 when Dr. Angelo
Naui called me in my cellphone and clarified to me if I had applied for the
monetization of my leave credits. I answered him no.
4. It came as a surprise that on January 6, 2018, Fe Malsi went to our
house in San Jose, San Pablo, Isabela and gave me Eight Thousand Pesos
(PHP8,000.00). I later found out the following Monday, January 7, 2019, that
she made the application on my behalf.

5. I later secured a copy of the disbursement voucher in my name in the


amount of Eight Thousand Give Hundred Seventy Six Pesos (PHP8,576.00)
attached as Annex “C”.

6. To my dismay, this has not been the only incident concerning my


forged signatures for applications and authorizations created in my name. The
attached documents are the most recent ones being that they are the only ones
I could immediately have produced.

7. Fe Malsi falsified all documents necessary to receive the benefits that


do not belong to her. She made false representations for her benefit and to the
detriment of her co-workers. To make matters worse, this has not only
happened once but had been a recurring incident.
8. If Dr. Angelo Naui had not called, I would not have known nor would I
have been informed that the benefits I have been so carefully collecting had
been already taken and had benefited someone else.
