MID- TRIMESTRAL ASSESSMENT

LEARNING FACILITATOR
PERFORMANCE- BASED ASSESSMENT:
“Basic Life Support and First Aid Training”

GOAL FOR THE ACTIVITY:

The activity is designed to train the Grade 12 students who are currently taking the course
subject, Disaster Readiness and Risk Reduction to a basic standard of First Aid, to impart a wide
spectrum of knowledge spanning the basic principles of immediate actions to be taken in
emergency situations, and consolidating such knowledge through the discussion and hands-on
practice of numerous common scenarios.

The project aims to achieve the following goals:

a. Learners possess and apply the skills and knowledge in Basic life support and First aid during
emergency situations.
b. Learners realize the importance of knowing Basic life support and First aid.
c. Learners asses an emergency situation calmly and quickly.

ROLE OF THE STUDENTS:

You are a member of Tacloban City Rescue Unit who will undergo a training on Basic life
support and First Aid.

AUDIENCE:

The audience for the orientation are all Grade 12 Senior High School Students of St. Therese
Christian Development Center Foundation Inc. taking Disaster Readiness and Risk Reduction.

SITUATION:

As their final class output, learners are expected to work together as a team to learn and have
simulations on Basic life support and First aid application to fulfill requirements for the learning area.
Learners will be given 4 hours from 8- 12 o’clock in the morning on September 10, 2019.

PRODUCT PERFORMANCE AND STANDARD:

PRODUCTS/OUTPUT/CONTENT

Learners are expected to submit an accomplishment report on the conducted activity.

STANDARDS AND CRITERIA:

Outputs and students will be rated based on the following standards:

o Participation on the Training 50 points


o Quality and Content of Accomplishment Report 30 points
o Timeliness/ Promptness 20 points
Total: 100 points
