I, MA. TERESA LEONORA DELA CRUZ, a Notary Public for and in


the City of Tacloban, do hereby depose and state:

1. That a typographical error was made on the following prepared


and notarized deeds of sale between Arsenio D. Gabon and the
following persons:

a. Deed of Sale in favor of Dionesio A. Bacaycay;


b. Deed of Sale in favor of Arcadio Gabon;
c. Deed of Sale in favor of Benito M. Gabon;
d. Deed of Sale in favor of Maria Jannice M. Gabon;
e. Deed of Sale in favor of Marcelo M. Gabon;

2. That the name “JULITA CABANGBANGAN GABON” was entered


as the name of the spouse of the seller, Arsenio Gabon, instead of
“JULITA CABANGANAN GABON”;

3. That the true and correct name of the seller’s spouse is in fact
“JULITA CABANGANAN GABON” as shown in her submitted
competent evidence of identity, hereto attached as Annex A;

4. That the above-mentioned Deeds of Sale have been corrected and


countersigned by me, the undersigned Notarizing Attorney;

5. That I hereby certify the truth of the foregoing facts and for
whatever legal purposes it may serve.

This certification is issued upon the request of Arsenio Gabon this
