II. Clinical Portrait of the Child:(summarize the physical assessment of the child to
narratively describe his/her clinical portrait)
Lorenz Mendoza, a 2yr old boy, lives in San Pablo Community wen he is assessed he has
a temperature of 36.2 °C at axillary site, Pulse of 80 bpm and Respiratory rate of 20cpm. In
general, his level of consciousness is Alert, his mood is pleasant as he response to our assessment
but has a flat affect on what we are doing to him. He weights at about 15kg and stands at 2ft and
11 inches tall.
His skin color is appropriate to race, hair distribution is even, his skin is most and warm,
he also doesn’t have edema or skin lesions that is present on his skin, and also do not have
birthmarks anywhere on his body and has a good skin turgor for about 3 seconds. His nails were
pink and has a rapid capillary refill for 2 seconds.
His head is 50cm in size which is an indication of Normocephaly, his both anterior and
posterior fontanelle is closed.
His eyes’ pupil sized for about 3mm and its equal, his iris color is black, his sclera,
cornea and conjunctiva were all clear. His vision is not identified yet if he is a far sighted or near
sighted but he does have a blink reflex.
His ears’ position is normal and it doesn’t have any presence of pain and wax build up.
His nose doesn’t have drainage, blockages, flaring and congestion on both nostrils, he has an
active sense of smell and the mucous membrane of his nose is moist.
His mouth has a pink mucous membrane, his tongue shape and movement is symmetric,
his saliva amount is normal, he already have 20 teeth and it is intact as well as his palate, his
swallowing is easy and he also has normal lymph nodes.
His breath sounds were clear, respiration is regularly even, chest expansion is
symmetrical, he doesn’t have cough and SOB (shortness of breath).
His heart sounds, both S1 and S1 were clearly audible, heart beat is regular, apical rate at
85bpm and apical rhythm is regular
. His abdomen is in round shape, his bowel sounds were present and active in all four
quadrants, if palpated his abdomen is soft, his diet is mixed feeding of solid foods and formula
milk, his toleration of diet is good, he has changes in appetite were he eats more that than he
does before, and he also has a recent weight gain.
He has sacral dimple on his back, his posture is straight, his both upper and lower
extremities are fully functional in ROM and his gait is balances since he walks actively.
All of the things that is stated below is the summary of Lorenz Mendoza’s clinical
portrait that has been observed and assessed during the day that he was visited and as he gets
along with the examiner even with a short period of time.
