Disaster Drill Scenario
Date of Drill: June 26, 2019
Location: Increment ll – Package 1 (Between 3A & 3B)

Drivers experiencing poor visibilities due to sand storm and poor road condition.
One very windy and dusty day on June 26, 2019, Three (3)
vehicles; AYC Pick-up and KOLIN Coaster which were passing on
Scenario opposite direction of the road and another KOLIN Pick-up which was
tailgating the Coaster between 3A & 3B Area At around 9:30 am. The
Three (3) vehicles collided to each other due to poor visibility and
tailgating.
The witness (Flagman) called the KOLIN Safety Officer assigned
to that area. KOLIN Safety Officer then immediately notifies his
Safety Supervisor. The HSE Supervisor immediately called the
Sinohydro Clinic and requested an ambulance. Safety Supervisor
reports the situation to his HSE Manager. KOLIN HSE Manager
then called AYC HSE Manager. AYC HSE Manager called their
clinic and requested an ambulance, KOLIN & AYC HSE
Managers notifies their Project Managers and SAPMT
representatives respectively.
Upon assessment, the two drivers (AYC Pick-up Driver and
KOLIN Coaster Driver) suffered major injuries as evidenced by
open wound and bloody head and both unconscious, the Coaster
passenger sustained minor injuries and the other KOLIN Pick-up
driver sustain minor contusion on the head. First Aid were given
immediately to the victims by the safety officers who responded
while waiting for the ambulance and cordoned off the incident
scene.
The drivers who suffered major injuries were transported with the
ambulance to the nearest hospital and the two passengers who
sustained minor injuries were brought to the site clinic for further
medical attention.

When the medical team arrived, assist the Victim to be brought to the
ambulance using a stretcher/Spine Board.
Document the incident and to be submitted to SAPMT officially.

TIMELINE
A coaster and a pick-up collide between 3A & 3B Area.
9:30 am The Flagman immediately called and informed the Safety Officer assigned to
that area.
9:30 am The Safety Officer notifies his immediate HSE Supervisor.
The Safety Supervisor called the Emergency Number (Sinohydro Clinic -
9:31 am 051-528-1427) for details of the incident and requested an ambulance and
inform his Manager about the incident.
The Safety Officer’s nearby came to help and provide first aid to the victims until
9:32 am
ambulance arrives and cordoned off the incident scene.
KOLIN HSE Manager called AYC HSE Manager about the incident, AYC HSE
9:33 am
Manager then called their clinic and requested an ambulance.
KOLIN & AYC HSE Managers notifies their Project Managers and

SAPMT representatives.
Sinohydro Clinic ambulance arrived to pick up the victim No.1 sustaining major

injuries and transported to the nearest Hospital.
AYC Clinic ambulance arrived to pick up the victim No.2 sustaining major

injuries and transported to the nearest Hospital.
9:38 am The two victim sustained minor injuries were transported to the clinic.
9:40 am KOLIN & AYC HSE team conducting investigation.
10:00 am 3rd party insurance company arrive to investigate the incident as well.
