Dear Ma’am/ Sir,

I, the undersigned name is sending you this letter as notification that I am allowing/
authorizing Mr. ARMAN ROE CALBALINAN BALADJAY, my older brother to process
and claim my Transcript of Records and Diploma in your office, I graduated last March
2019 with the Degree of Bachelor of Science in Information Technology (BSIT)

I won’t be able to do it personally because of an important reason that is why I have asked
him to process and claim my Transcript of Records and Diploma. I am also giving him the
right to sign any papers that have a connection with the processing of my Transcript of
Records and Diploma.
