RELEASE, WAIVER AND QUITCLAIM

KNOW ALL MEN BY THESE PRESENTS:

I, ______________________________, of legal age, Filipino, married and


presently residing at ______________________________, hereby confirm my lawful
separation from ______________________________ (Company) with principal place
of business at ______________________________, effective the closing of the office
hours of _______________, and further acknowledge having actually received from the
Company the amount of PESOS: ____________________ (Php _______________),
representing the full and final settlement of all my remaining salaries and employment
benefits, including the proceeds of my Retirement Benefits, in accordance with the
employment contract I have entered into with the Company and existing laws, as well as
any and all claims which I may have against the Company arising from such salaries,
benefits and Retirement Benefits, and declare

1. I have no more right or interest of any kind whatsoever from the Company and its
successors-in–interest, including its directors, officers, employees and agents,
arising from the such salaries, benefits and Retirement Benefits, and any and all
claims in relation thereto, absolutely and unconditionally releasing, remising, and
forever discharging the Company and its successors-in–interest, including its
directors, officers, employees and agents, from any and all action, expected, real
or apparent, sum of money, damages, claims and demands whatsoever, which in
law or equity I ever had, now have, or which I, my successors and assigns
hereafter may have by reason of any matter, cause or thing whatsoever, up to the
time of this Release, Waiver and Quitclaim;

2. I agree that this Release, Waiver and Quitclaim may be pleaded in bar to any suit
or proceeding to which either I, or my heirs and assigns, may have against the
Company, and its successors-in-interest, including its directors, officers,
employees and agents; and

3. I declare that I have read and understood this Release, Waiver and Quitclaim,
with full knowledge of my rights under the law.
