KNOW ALL MEN BY THESE PRESENTS:

I, , of legal age, Filipino citizen, with address at, hereby name, constitute, and

legal age, Filipino, with address at _________________, to be my true and lawful attorneys-in-
fact, for me and in my name, place and stead, to do and perform the following acts and
things, namely:

1. To apply, process, procure, follow-up and receive any document/s pertaining to my


application for the issuance of my Authority to Print receipt for my business, named
ALIKHAN GENERAL MERCHANDISE, before the proper RDO of the Bureau of
Internal Revenue (BIR).

2. To apply, process, procure, follow-up and receive any document/s pertaining to my


application for the issuance of my business permit for my business, named ALIKHAN
GENERAL MERCHANDISE, before the City Hall of Silang Cavite.

3. To make, sign, and execute, other writings of whatever nature or kind, with any
and all third persons, entities or concerns, as well as RECEIVE, ACCEPT, and
COLLECT, the above-cited application, upon terms and conditions acceptable to my
said Attorney-in-Fact;

4. To pay any necessary fees for the completion and issuance of my Authority to Print
receipt with the BIR, as well as my business permit with the City Hall of Silang Cavite,
and other applicable government agency/ies, both for my business, named ALIKHAN
GENERAL MERCHANDISE,

5. To do and perform and all other acts and deeds necessary to undertake the execution
and finalization of the foregoing application and issuance of the above-described
applications.

HEREBY GIVING AND GRANTING unto my said attorneys-in-fact full power and
authority to do and perform all and every act requisite or necessary or proper to be done in
or about the premises, as fully to all intents and purpose as I might or could lawfully do if
personally present, and hereby ratifying and confirming all that my attorneys-in-fact shall
do or cause to be done under and by virtue of these presents.
