1. Register showing the computation of the allocable surplus in form A


2. Register showing the set on and set off the allocable surplus in form B

Maternity benefit Act:-

1. Muster roll in form A

The Punjab industrial establishment


(National and festival, casual and sick leave) Act 1965:

1. Register of festival, holiday, casual & sick leave in form B

Payment of wages act:-

1. Wage register
2. Register of fines in form I
3. Register of deduction in form II
4. Register of advance in form III
ESIC:-

1. Attendance register/Muster roll


2. Salary/wage register/payroll
3. EC (Employee’s & Employer’s contribution) statement
4. Employee’s register U/R 32 (form 7)
5. Accident registers U/R 66
6. Return of contribution (RC- form 6)
7. Return of declaration form (RDF- form 3)
8. Receipted copies of challans
9. Books of account viz cash/ bank, expense register, sales/purchase register, petty cash book,
ledger, supporting bills & Voucher, a/w delivery challans (if any)

Equal remuneration act:-

1. Register of equal remuneration in form D

Employee provident fund:-

1. Attendance register/Muster roll


2. Salary/wage register/payroll
3. EC (Employee’s & Employer’s contribution) statement

Punjab welfare fund:-

1. Register of wages in form A


2. A Consolidated register of unclaimed wages and fines in form B

Workmen’s compensation act:-

Every Employer shall display the Abstract of the act and rules in English & Hindi or any other
language understood by majority of worker in Form HH.

The industrial employment (Standing Order)act:-

Copy of certified Standing Order at the main entrance of the establishment


Contract labour act:-

1. Register of contract in form 8 by principal employer


2. Register of person employed in form 9 by the contractor
3. Employment card in form 10 (to be issued to workmen)
4. Muster roll in form 12
5. Register of wages in form 13
6. Wage slip in form 15
7. Register of deduction for damages or loss in form 16
8. Register of fines in form 17
9. Register of advance in form 18
10. Register of over time in form 19

Note: the following register to be maintained by the contractor as a employer where


establishment is governed by the payment of wages act 1936 and minimum wages act 1948

1. Muster roll
2. Register of wages register of deduction
3. Register of over time
4. Register of advance register of fines
5. Slip

Every contractor shall maintain and safe the records:-

• up to 3 years of the last entry


• shall be produce on demand
• shall be placed at office or nearest building with in the precincts of the establishment where
work is going on or with in 3 km radius of establishment

Display:-

1. Every contractor shall display the Abstract of the act and rules in English & Hindi or
any other language understood by majority of worker.
2. Notice showing rates of wages, hours of work, wages period, date of payment, name &
address of the inspector at the main entrance of the establishment where work is going
on.
