Elvira Marcelo has petitioned the Municipal Agrarian Reform Office Cluster 2 to reallocate the application for a Certificate of Land Ownership Award from the heirs of Patricio Bernardo, represented by Juliana Bernardo Cruz, to her name. Juliana Cruz issued Elvira Marcelo a waiver of rights. Elvira Marcelo also submitted an affidavit stating that the heirs of Patricio Bernardo transferred their rights to apply for the CLOA to her. PETITION
COMES NOW the petitioner and before the Officer of the Municipal Agrarian Reform
Office Cluster 2 respectfully states:
1. That the petitioner is of legal age, Filipino, Single and a residence of Brgy. Garlang
San Ildefonso, Bulacan.
2. That JULIANA BERNARDO CRUZ legal representative of the Heirs of Patricio Bernardo
issued a waiver of rights in favor of the petitioner.
3. That the petitioner has given the rights by the Heirs of Patricio Bernardo represented
by Juliana Bernardo Cruz to apply for the Certificate of Land Ownership Award under
the petitioner’s name.
4. That submit herewith are the following papers in support of this petition;
a. Special Power of Attorney from the heirs of Patricio Bernardo for Juliana
Bernardo Cruz as their attorney-in-fact.
b. Deed of Waiver of Rights
c. DARAB Case No. R-03-02-0367’00
d. Others.
PRAYER
WHEREFORE, it is respectfully prays that the Honorable Municipal Agrarian Reform
Office GRANT the instant petition for reallocation of the application for the CLOA of Patricio
Bernardo in favor of the Petitioner.

I, ELVIRA MARCELO, Filipino of legal age, single and a resident of Brgy. Garlang, San
Ildefonso, Bulacan, Philippines after having been sworn in accordance with the law depose and
state;
1. That Mrs. Juliana Bernardo Cruz is the legal representative of the heirs of Patricio
Bernardo by virtue of Special Power of Attorney.
2. That Mrs. Juliana Bernardo Cruz issued a waiver of rights in favor of Elvira Marcelo
3. That the heirs of Patricio Bernardo represented by Juliana Bernardo Cruz transferred
their rights to me to apply for the Certificate of Land Ownership Award.
4. That I am executing this affidavit in connection with my petition of reallocation for
the application of the Certificate of Land Ownership Award for the heirs Patricio
Bernardo represented by Juliana Bernardo Cruz.
