Use this form to notify Compliance Monitoring and Enforcement Department (CMED) regarding implementation of
modification/s to, new program/component or change of bingo program, conversion of bingo cards or installation of
Bingo Link for the traditional bingo game offering at the gaming site. Please accomplish this form by providing correct
and complete information. The Operator shall submit this form to CMED at least one (1) business day prior to
intended date of implementation. Cut-off for submission of notifications shall be 4:00 P.M.
PLEASE NOTE THAT PAGCOR RESERVES THE RIGHT TO CONDUCT POST-AUDIT OF THE NEW,
MODIFICATION TO, CHANGE OF BINGO PROGRAM, CONVERSION OF BINGO CARDS OR BINGO LINK
AT THE PAGCOR-APPROVED GAMING SITES HOSTING BINGO GAMES.

e. Payout percentage (The computation of payout percentage for the each game and session)

We confirm and undertake to do the following, as applicable:
The attached documentation/s for new or modified bingo program which include prize payout scheme/s,
bingo game rules and mechanics, among others, are consistent with standards for bingo program
prescribed under Regulation 10 of Gaming Site Regulatory Manual (Bingo Games).
The prizes in each bingo game in any bingo session of the bingo program shall be at least thirty percent
(30%) of the projected gross receipts from the sale of package cards and extra cards.
The Operator warrants that new bingo program/s or modification/s made to bingo program/s is/are not
inconsistent with the PAGCOR’s prescribed guidelines on bingo programs.
Bingo program along with its modification/s, if any, shall be made available to all players prior to the start
of the bingo operations for the day.
The Operator is committed to abide and pursue the amendments as initiated and notified to PAGCOR.
No amended bingo program or change of bingo program shall be implemented until the jackpot prize in a
current regular bingo game or special game with high ball level is won. High ball level refers to the
number of balls at which the probability of winning or chances the prize will be won is great.
The progressive jackpot prize with fixed ball bracket of a particular game shall be maintained.
Pre-approved prize payout scheme/s and game rules and mechanics shall apply for each change of
bingo program.
The Operator agrees that conduct and operation of Bingo link shall be governed by the procedural
guidelines as prescribed in Part II – Bingo Games via Closed Circuit Television (CCTV) and Computer
Links of B. Traditional Bingo under Regulation 18 of Gaming Site Regulatory Manual (Bingo Games).
The implementation of the new bingo program/component, modified bingo program or change of bingo
program or installation of Bingo Link shall be carried out in close coordination CMED.

copies may be submitted directly to the CMED office or scanned copies of the documents may be emailed
using the e-mail addresses indicated below:

If submission of this notification form is done thru e-mail, the Operator should ensure that it has been duly
received and acknowledged by CMED. The Operator shall be notified thru e-mail confirming receipt of this
notification form. If no e-mail confirmation is received, the Operator should submit the hard copy of the
notification form directly to CMED. If implementation falls on a non-working day (i.e., Saturday, Sunday and
holiday), CMED shall confirm receipt of the notification form on the working day immediately following the
non-working day.
