TERMS AND CONDITIONS

Concentrix has allow Mr./Ms. ___________________ as participant employee/vendor of _____________


on December 14, 17 & 18, 2018 during the Holiday Bazaar activity in Concentrix. The employee/vendor
will insure the health and sanitation practices of their respective products and / or services and will be
responsible for safekeeping and management of their products and staff during the event period.
Employee/Vendor must impose disclaimer statement of possible allergen content of their products to
avoid complaints and other possible health issues/ claims from the customers.

Employee/Vendor has read and understood the terms and conditions set herein. Employee/Vendor
further acknowledged the inherent potential risks/hazards/exposure associated with this type of activity
and shall adopt stringent measures to prevent/mitigate any such risks/hazards/exposure.

For the above stated considerations, employee/vendor hereby freely and voluntarily release and
discharge Concentrix from any action/claims for damages, sum of money and other demands arising from
this activity or that may be filed in a court of law/judicial body for failure/non-compliance with the above
stated terms and conditions. Employee/Vendor assumes full responsibility of the products and services
provided/sold.
