Near Miss Report 18

Nikhil Sambharkar observed a gas leak around a reversing valve after opening its stop cock slightly during a servicing job in Cellar #1. He immediately closed the stop cock when he noticed the leak, but a blast still occurred from the reversing valve, though it did not cause any burn injuries. After reversing valve servicing job, when stop cock of HW03(E) was slightly opened by
Nikhil Sambharkar gas leakage was observed around reversing valve, so stop cock was
immediately closed, but blast took place from the reversing valve and no burn injury was
occurred.

Remedial measures suggested:

1. Proper setting & greasing of reversing valve assembly to be done.


2. PEP talk & SOP training should be given on regular basis.
