Christian greetings!

We, the MAPEH Department would like to ask approval from your good office that we may be
allowed to conduct the Grand Launch of our Zumba Dance Craze for our students, teachers and
satkeholders to improve their overall physical and mental fitness. This is one way to launch
generated project that support the needs of our students in our department. The proposed date of
this Zumba Dance activity implementation will be on this coming Thursday or Friday November 15
and 16, 2018.

Should there be any inconvenience with the schedule, we are willing to adjust the period of our
Zumba Dance Program. Thank you for your usual concern regarding our school undertaking.
