I, JIORILYN BROCA, of legal age, married, and resident of Purok 1, Bacusanon,


Pangantucan, Bukidnon, Philippines, after having been sworn to an oath in accordance
with law, hereby depose and say;

1. That I am the authorized representative of ARTCHIE A. BROCA to claim his


TESDA Allowance for Pipe Fitting NC II and toolkit as evidenced by his
authorization letter dated June 27, 2018;

2. That I am executing this affidavit to attest to the veracity of the foregoing for
whatever legal purpose that this may serve.
