1. Description of activities you engaged in this period.


- Uploading photos from the Gastroenterology physician’s office.
- Indexing photos from the Gastroenterology physician’s office.

a) Describe your activities in some detail.


Some of the activities that I performed during this period of time include the re-
training on the DCS system. I performed this activity in order to make sure that I
uploaded and indexed the photos from the Gastroenterology Physician’s office in the
right way. In addition, I checked all the prepayment deposits from the requested of
medical documents in order to double check that every person who asked for medical
documents did the prepayment deposits in order to authorize the release of this medical
documentation.

b) Include your reactions to these activities and to your clients and peers.
My reactions were very professional, but, internally, I was so happy for all the
processes that I was watching and involved in; it was very exciting to actually see what
the real world of my major was like.

c) Were you supervised during these activities?


Yes, I was supervised during these activities. The majority of the time Ms.
Squadrito, but, if I was in an employee’s office or in my office, then I was without
supervision.

1. If yes, describe the supervision and include your reaction to your supervisor
and supervision.
The supervision was very professional, and, at the same time, I felt their
supervision was great since they always made me feel so welcome. My reaction to
Ms. Squadrito was professional and very friendly and always made sure I was
understanding everything.

2. If no, describe your activities when you were not supervised and your
reactions.
When I was without supervision, I had the same behavior as if I was being
supervised because I wanted to make sure everything I did was done effectively
and responsibly. I had a great respect for Ms. Filipovich because she was the one
who took me and it was never my intention to lose the trust and faith that she gave
to me.

2. What did you expect to learn through these activities?

I learned about the usage of the DCS system and how to avoid errors during the
uploading process of the patients’ information. In addition, I learned the scanning,
and indexing of these documents.

I did not have any direct contact with the clients.

3. What exactly did you learn through these activities?

I learned about the usage of the DCS system and how to avoid errors during the
uploading process of the patients’ information. In addition, I learned to perform the
processes of scanning and indexing of these documents.

4. Can you identify any initiatives as required by the organization’s performance improvement
planning or as required by the regulatory bodies, such as CMS?
a) If so, what are they and who is requiring them?
Yes, I can. The maintaining of medical records for X or Y years are requirements
that New York and Federals health laws demand and; therefore, every hospital in the U.S.
most follow and respect these laws and regulations.

b) If you can't identify do you see an opportunity for performance improvement?


N/A
5. What activities do you expect to be engaged in during the next 40 hours of internship work?
I expect to be engaged in the usage of the computer named KAR in order to make copies
of medical documents not older than 20 years. In addition, I expect to be engaged in the mailing
notifications of orders cancellations due to the no prepayment deposit for medical documents
requested.

6. What do you expect to learn during the activities of the next 40 hours of internship work?
I expect to learn how to use the KAR. In addition, I expect to learn how the employees
send the notification of order cancellation for their medical documents.

7. What do you WANT to learn? Have you communicated this to your supervisor?
I want to learn about the usage of the computer named KAR and how to send the order
cancellation for medical records. Yes, I did communicat this to my supervisor.
