Findings
i. Valves item 10-01 received without actuator from Matco Jakar.
ii. Deep scratch at both seat, seat insert and ball due to foreign materials.
iii. No traces of metal foreign materials in valve closure as well as cavity (Could have been
removed after testing before dispatching the valve to MSB Kota Damansara)
iv. No scratches on the other side of closure for both ball and seat.
v. Seat insert cannot be polish or lapped due to high depth of scratches

Root cause Analysis


i. Scratches is due to hard metal object being pressed in between seat contact areas and
ball during movement or valve stroking.
vi. Source of hard foreign metal materials: Process Medium (Water) during testing

Cronology of Event
- 25th October 2018 : Valve arrived from Matco Jakar to MSB
- 26th October 2018: Perform Internal Testing on Valve
LPGT Test performed side A: SATISFACTORY
HPGT Test performed side A: SATISFACTORY
LPGT Test performed side B: UNSATISFACTORY
HPGT Test performed side B: UNSATISFACTORY

- 2 November 2018 : Dismantle the valve and perform inspection and root cause
analysis.
