citizen, with residence and postal address at No. 9039 Bo. Obrero, Minuyan
Norzagaray Bulacan, after having been duly sworn to in accordance with
law do hereby depose and say:

1. That I am a registered nurse of the Republic of the Philippines;

2. That I had been employed at Estrellas Nex-Care as Staff Nurse from July
1, 2009 up to September 15, 2011; Copy of my Certification issued by Ms.
Marcia Teresa Santos Estrellas, is hereto attached and forming integral
part hereof;

3. That I hereby attest to the authenticity and genuineness of the document


and I hereby agree to be liable and answerable to perjury and /or
falsification of public document in the event that the document is found
not in order;

4. That I am executing this affidavit to attest to the truth and veracity of the
above foregoing facts and for all other legal and purposes it may serve.
