PETITION FOR COMMISSION


AS NOTARY PUBLIC

Petitioner, to this Honorable Court, respectfully alleges that:

1. She is of legal age, having been born on 30 September 1984,


married, Filipino, presently residing at No. 1 Sta. Catalina Street Villa
Regina Subdivision, Angeles City, with telephone No. 888-68-32; 888-68-
55 and 625-39-05, with business address at 167 Sto. Rosario St., Sto.
Domingo, Angeles City. The location sketch of petitioner’s office is
attached marked as Annex “A” hereof;

2. She has been admitted to the practice of law in the Philippines as


shown by the Certificate of Membership in the Philippine Bar, which
contains the petitioner’s Roll of Attorney No. 60590. A copy of the
Certificate of Membership in the Philippine Bar is attached and marked as
Annex “B” hereof;

3. She is a member of good standing of the Philippine Bar as shown


by the clearance issued by the Office of the Bar Confidant of the Supreme
Court, a copy of which is attached and marked as Annex “C” hereof;

4. She is likewise a member of good standing of the Integrated Bar of


the Philippines. A copy of her Certificate of Lifetime Membership is
attached as Annex “D” hereof. Also attached herewith as Annex “E” is the
Certificate of petitioner’s good moral character issued by Attys. Peter Paul
S. Maglalang and Ponciano V. Dela Cruz, Jr., President and Treasurer,
respectively of the IBP Pampanga Chapter;

5. She has paid her professional tax for the year 2014 as shown by
the Professional Tax Receipt No. AC-3420323 dated 3 January 2014,

issued by the City of Angeles which is attached and marked as Annex “F”
hereof;

6. Her Tax Identification No. is 255-516-969-000;

7. She has never been charged nor convicted of any crime involving
moral turpitude;

8. She has all the qualifications and none of the disqualifications for
the Office of Notary Public;

9. She has no commission as notary in any other jurisdiction;

10. She has attached hereto in a separate sheet of paper specimen


of her official signatures and marked as Annex “G” and made integral part
hereof; and

11. She has likewise attached hereto her three (3) passport-size color
photographs each bearing her signature at the bottom part thereof.

WHEREFORE, petitioner respectfully prays that she be granted a


notarial commission in and for the City of Angeles for the year 2014 and for
the term ending in 2015.
