This letter is to inform you that you have failed to respond to my credit dispute letter that I
sent to you certified mail on (date letter#1 was sent). As a consumer, I have rights under the FCRA
and Federal Law mandates that you must respond to my dispute letter within 30 days of receipt of
my dispute letter.

I am attaching a copy of the dispute letter that I sent you via certified mail asking you to
verify the accuracy or remove the unverified accounts listed below. According to the USPS you
received my dispute letter on (date Credit Bureau received letter#1).

You failed to respond to my dispute letter and you failed to verify that these accounts belong
to me before the end of the 30-day period beginning on the date that you received my dispute letter.
Under 15 U.S.C. § 1681i, of the FCRA all unverified accounts must be promptly deleted from
my credit file. The files listed below are inaccurate and unverified and I am requesting that
you delete all of them from my credit report immediately.

You are also required to send me a copy of my credit file showing that these items have
been removed. . Please provide me with a copy of an updated and corrected credit report showing
these items removed.

I demand the following accounts be removed immediately.
