AFFIDAVIT OF LOSS
I, Juan De La Cruz, of legal age, single, and a resident of #34 Upper
LukeWright Street, Poblacion 2, Dumaguete City, having been sworn in
accordance with law, hereby depose and state that:
1. I am an employee of Jollibee Foods Corporation at Perdices Street,
Dumaguete City, Negros Oriental and at the same time a duly licensed
driver;
2. On November 10, 2018 at around eleven o’clock in the evening, while
I was at the Ceres Terminal at Dumaguete City, I have lost my wallet
which I usually place at the side pocket of my back pack;
3. Inside my wallet are my Driver’s License and an ATM Card issued by
Land Bank of the Philippines, Dumaguete Branch;
4. Despite my diligent effort of searching my wallet and my Driver’s
License and ATM Card, I could not find them of which I would believe
are now lost beyond recovery;
5. My Driver’s License has not been confiscated by the LTO, Police or
other traffic enforcers for any traffic violation; and
6. As such, I am executing this Affidavit of Loss to attest to the truth of
the foregoing and to support my application for the issuance of a new
Driver’s License and a new ATM Card, in lieu of the ones what were
lost.
IN WITNESS WHEREOF, I have set my hand this 12th day of November
2018, in Dumaguete City, Negros Oriental, Philippines.

DEED OF ABSOLUTE SALE
KNOW ALL MEN BY THESE PRESENTS:
This DEED OF ABSOLUTE SALE is made, executed and entered into by:
Juan Dela Cruz, of legal age, single, Filipino, and with residence and
postal address at #34 Upper LukeWright Street, Poblacion 2, Dumaguete City,
Negros Oriental, hereinafter referred to as the SELLER
-AND-
Maria Clara, of legal age, single, Filipino and with residence and postal
address at #70 Burgos Street, Barangay Ubos, Bayawan City, Negros
Oriental, hereinafter referred to as the BUYER.
WITNESSETH;
WHEREAS, the SELLER is the registered owner of a parcel of land
with improvements located in Mabuhay, Sta. Catalina, Negros Oriental and
covered by Transfer Certificate of Title No. FV-19883 containing a total area
of EIGHT THOUSAND SQUARE METERS, more or less, and more
particularly described as follows:
TRANSFER CERTIFICATE OF TITLE NO. FV-19883
"A parcel of land covered by Original Certificate of Title No. 19883 and
known as Lot No. 979, Pls790-D, situated in Mabuhay, Sta. Catalina, Negros
Oriental. Bounded: on the North along line 5-1 by lot 978, Pls-790-D; on the
East and Southeast along line 1-2 by National Road; on the South along line
2-3 by lot 980, Pls-790-D; on the West and Northwest along line 3-4-5 by
Sulu Sea, containing an area of EIGHT THOUSAND (8,000 sq. m.) SQUARE
METERS, more or less and under Tax Declaration No. 99-15-011-00305.”
WHEREAS, the BUYER has offered to buy, and
the SELLER has agreed to sell the above-mentioned property for the amount
of EIGHTY THOUSAND PESOS (₱ 80,000.00) Philippine Currency;
NOW THEREFORE, for and in consideration of the sum EIGHTY
THOUSAND PESOS (₱ 80,000.00) Philippine Currency, hand paid by the
vendee to the vendor, the SELLER DO HEREBY SELL, TRANSFER, and
CONVEY by way of Absolute Sale unto the said BUYER, his heirs and
assigns, the certain parcel of land together with all the improvements found
thereon, free from all liens and encumbrances of whatever nature including
real estate taxes as of the date of this sale.
