materials by hand while climbing top and bottom to avoid sliding or falling.
Tools and materials should be carried up using a
tool belt or a rope to pull things up once you’ve
stopped climbing.
Never load ladders beyond their rated capacity,
including the weight of the worker, materials and
tools.

vapors, or potentially injurious light
radiation. These hazards are present
when doing a variety of task on the
job site such as welding, chipping,
grinding, masonry work, sanding,
woodworking and drilling.
5. Head Hard hats are commonplace at the

workers a number of hazards such as
falling and flying objects, electrical
shock and other impacts.
Term Definition
1. Confined A space that has the following criteria:
Space 1. Large enough and so configured that an employee can physically enter
2. Limited or restricted means for entry or exit
3. Not designed for continuous employee occupancy

Description Purpose
1. Work Permit 1. Identify the scope of a specific task and where the work will be conducted.
2. Communicate activities throughout the facility through a PTW review meeting that is held
daily.
3. Identify hazards and controls associated with the work task
4. Identify the personnel who have oversight responsibility
5. Identify the personnel performing the work
6. Assign responsibility for various tasks
7. Identify potential issues with other work activities through the PTW review meeting
8. Identify energy isolations/lockout/tagouts that will be in effect for the work that is being
conducted
9. Verify adequate control over the return to normal operations
2. Tool Box 1. Ensure that all other persons involved in the task fully understand the scope of the work,
meeting the identified hazards and associated controls
3. Job Safety 1. Identify hazards and provide actions to mitigate all known hazards with a particular task.
Analysis (JSA) 2. It is usually a Level 1 Risk Assessment performed by a single Authority. Level 2 Risk
Assessment is performed by a team.
Types of Permits
Permit Purpose Examples/Notes
1. Hot Work required for work involving the use Electrical welding and use of welding machine,
Permit of a flame or other sources of Flame cutting,
ignition Grinding (produces sparks)
2. Cold Work required for potentially hazardous Chemical cleaning or use of solvents,
Permit work not covered by other types of Handling of hazardous substances (e.g.
work permits toxic/corrosive chemicals, asbestos, etc),
Any painting activity,
Heavy lifts
Erecting or dismantling scaffolds,
Any non‐routine and potentially hazardous
activity,
Any activity requiring specific control measures to
confirm safety
3. Electrical required for work on electrical Work involving the installation or repair of
Permit systems where there is a possibility electrical conductors,
of contacting energized electrical Connection or disconnection of electric motors,
conductors Reaching into any panel, transformer or other
electrical enclosure which may have energized
circuits, capacitors, wiring, etc.
Work on instrumentation, instrument panels, or
telecom equipment,

4. Confined required to allow personnel to enter A confined space entry permit allows access and
Space Entry a confined space such as a trench, inspection only.
Permit tank, vessel, etc. When hot work, cold work, electrical work, and/or
energy isolation work is carried out in a confined
space, an additional permit(s) (as required by the
type of work) must be applied for and cross
referenced to the confined space entry permit
5. Excavation required for any excavation or If ground disturbance will be conducted utilizing
Permit ground disturbance activity that mechanical equipment such as trenchers,
involves digging, trenching, backhoes, bulldozers, etc. a designated spotter

ground adequate observation is being conducted to
prevent contact with overhead power lines and to
observe for any unusual objects or sounds from
the excavation. This person will be dedicated to
this purpose and listed by name of the permit
Procedure
1. A Work Permit must be filled out and approved at the beginning of each workday before work commences. The
work permit is a daily permit and will need to be signed off at the end of each workday with comments related
to the day’s progress.
