I , , of legal age, Filipino, and a resident of, after having been duly sworn to in
accordance with law hereby depose and say:

That, I am the licensed owner of one Cal. 32 Pistol with serial no.
522077;

That, sometime in the year 2013, I discovered that the license of


the said firearam was already lost in my cabinet.

That, I exerted diligent efforts to locate the same but to no avail;

That I am executing this affidavit in order to attest to the truth of the


foregoing and to establish the fact of the loss of my firearm license.

FURTHER AFFIANT SAYETH NAUGHT.
