Dear Dr. Magpantay

I, Ms. Judy Ann May Sapico, writing this application letter to prove my objective, to be
considered for the vacant position as a nurse for the NURSE DEPLOYMENT PROGRAM offered by the
Department of Health.

I have been working at Rizal Provincial Hospital System Antipolo Annex I at Pediatric ward for
almost 2 years and I am now currently working at Rizal Provincial Hospital System Antipolo Annex II at
the same ward since August 2017.

I am also a member of Parish Youth Ministry at San Antonio De Padua Parish-Dalig Antipolo,
which in every activity I act as a medic in case of some participants need my help regarding to medical
field and give them first aid and some interventions.

I can adapt easily to any given situation and can handle various medical situations professionally
and efficiently using my own knowledge, experience and any available resources. I am willing to work
within Region IV-A (CaLaBaRZon) and work beyond office hours if needed.
