Obsessive Compulsive Inventory

This document describes the Obsessive Compulsive Inventory (OCI), a psychological assessment used to measure obsessive-compulsive disorder (OCD) symptoms. The statements below refer to experiences that many people have in their everyday lives. In
the column labelled FREQUENCY, CIRCLE the number next to each statement that best describes
how FREQUENTLY YOU HAVE HAD THE EXPERIENCE IN THE PAST MONTH. The numbers in this
column refer to the following labels:

0 = never 1 = almost never 2 = sometimes 3 = often 4 = almost


always

Then, in the column labelled DISTRESS, CIRCLE the number that best describes HOW MUCH
that experience has DISTRESSED or BOTHERED YOU DURING THE PAST MONTH. The
numbers in this column refer to the following labels:

0 = not at all 1 = a little 2 = moderately 3 = a lot 4 = extremely
