PLANTILLA JUSTIFICATION
Administrative Aide VI/ Utility Foreman This position will maintain the orderliness of the
documents and cleanliness of the facilities in the office.
Also, he/she will send the communication letters and
important documents to other offices.
Administrative Assistant II The position will be responsible in monitoring of the
supply of the office delivered by Procurement Office or
direct purchase from stores as indicated in the year’s
Program Procurement Management Plan as well as the
allocated resources usage in the office. He/she will also
facilitate the direct purchase of goods for the office and
its delivery.
Senior Administrative Assistant II This position shall be responsible of preparing data
processing by directing entry to the computer, review/edit
files & output data based from sources documents. It will
maintain backup files in external and cloud storage. It will
also prepare reports and other data required by the
Department Head and other offices.
Administrative Assistant IV This position functions process Employee Daily Time
Records, Leaves and Locators forms and submits to the
CHRMD. He/she processes scheduling of the service
vehicles and drivers as directed by the Department Head.
He/she also monitors daily attendance especially on Flag
Raising Ceremony, internal office meetings and other
special events.
Administrative Officer II This position will have function of creating, maintaining,
and updating the database of the office. Also trains the
other staff in using the database of the office.
Administrative Officer IV This position will have a function of monitoring of
programs and services. He/she will be responsible in
training other employees to the daily functions.
Supervising Administrative Officer This position shall be responsible for the development,
integration and implementation of policies and programs
concerning Social Services.
Disability Affairs Assistant This position will perform routine clerical typing jobs,
keeps professional records and files. He/she will also
performs correspondence and coordinates with other
professional staff in terms of case management.
Disability Affairs Officer I The position functions as case manager in providing social
welfare services for walk-in clients or as endorsed in the
office by other agencies for PWD issues and concerns.
This includes but not limited to preparation of reports
(Social Case Study Report, Child Study Report, Home
Study Report, etc.), facilitate coaching sessions, referral to
other agencies and recommend eligibility to psychosocial
services.
Disability Affairs Officer II The position functions as the facilitates in implementing
psychosocial programs and observance of mandated
National Celebrations which would help in awareness,
empowerment, non-discrimination and community
inclusion of PWDs. He/she will also assist in the case
management of clients endorsed to the office.
Disability Affairs Officer III The position functions as supervisor of Disability Affairs
Officers in the performance of their duties and assigns
workload to ensure proper distribution of work. He/she
will also conduct regular supervisory conferences for staff
development and more effective program
implementation. This position will also conduct technical
assistance and training to other Disability Affairs Officers
Disability Affairs Officer IV This position functions in executing and implementing
policies and renders administrative decisions on matters
within delegated authority by the Department Head.
He/she will lead in implementation of programs, projects
and activities as indicated in the year’s Annual Investment
Plan (AIP) for PWDs.
Cook II This position will be responsible in preparing monthly
menu, keeps the kitchenware facilities and dining area
clean and in order, supervision of food storage, regular
marketing and reporting of food cost.
Cook II (Reliever) This position will be responsible in preparing monthly
menu, keeps the kitchenware facilities and dining area
clean and in order, supervision of food storage, regular
marketing and food cost in times that the Cook II could
not come to work.
House Parent II This position functions as being parent to children &
youth by instilling discipline (rules and regulations),
undertake home management (hygiene, sanitation,
physical order) and supervising housekeeping (cooking,
dishwashing, laundry, gardening and beautification)
House Parent II (Reliever) This position functions as being parent to children &
youth by instilling discipline (rules and regulations),
undertake home management (hygiene, sanitation,
physical order) and supervising housekeeping (cooking,
dishwashing, laundry, gardening and beautification) in
times that the House Parent II could not come to work.
House Parent III This position will be responsible in coordinating with the
Social Worker and allied professionals in the child’s
rehabilitation. He/she will also execute educational, skills
and vocational training programs for children as
recommended by the case manager. The programs
include but not limited to basic literacy, basic carpentry,
arts & craft, cookery, talent and sports development, etc.
House Parent IV This position will be responsible in supervision of home
life services and determines the duties of other house
parents in the time schedule. Also, he/she will attend
meeting and case conferences. Moreover, he/she will
review and enrich for the care and treatment of the child
Laundry Woman This position will maintain the cleanliness of the clients’
clothes, mattresses, pillow cases and office curtains, etc.
Photographer This position shall be responsible in documenting the
supplies, materials, and equipment for all divisions. Also,
he/she will assist in the documentation of the meetings,
events and daily office services. He will help in the
packaging of the SSD programs and services in print
and/or online.
Planning Officer I The position will have the function monitoring the Office
daily psychosocial services by preparing the daily and
weekly accomplishments report which will submitted to
the Planning Officer III. He/she will also assist in the
preparation of reports as needed by the Department
Head and/or other concerned offices.
Planning Officer II This position will monitor the accomplishment of each
programs implemented per division to achieve the full
utilization of the budget and target beneficiaries indicated
in the year’s Annual Investment Plan. He/she will also
provide technical assistance to the employees to achieve
the targeted budget and beneficiaries. He/she will also
assist in the preparation of the Budgetary Documents
(AIP, LBP Forms, PPMP, etc.)
Planning Officer III This position shall be responsible in preparing the
mandated yearly plans (AIP, LBP Forms, and PPMP) and
other pertinent budgetary plans to be submitted to
Department Head. He/she will consolidate the reports per
division for their monthly, quarterly and yearly
accomplishment Reports. Moreover, he/she will facilitate
the planning of programs, projects and activities of the
office per divisions. He/she will also facilitate the Program
Review of the services in the office.
Planning Officer IV This position shall be responsible in the planning,
developing and monitoring of the programs, projects and
activities of the office.
Psychologist I The position function as psychological service provider to
(based on Republic Act 10029 or Philippine walk-in clients or as endorsed in the office by other
Psychology Act of 2009) agencies. The psychological services include but not
limited to psychological interventions (psychotherapy,
psychological debriefing, psychosocial support, group
processes, etc.), psychological assessment (clinical
interview, use of psychological tests, etc.) and
psychological programs (development, planning,
monitoring, implementation and evaluation of
psychological treatment for individuals and/or groups)
Psychometrician The position will be responsible in assisting the
(based on Republic Act 10029 or Philippine Psychologist in providing psychological assessment to
Psychology Act of 2009) clients by administering, scoring and interpreting
psychological test and by preparing its written report.
He/she will also conduct preparatory intake interviews to
clients for psychological intervention sessions.
Social Welfare Assistant This position will perform routine clerical typing jobs,

professional staff in terms of case management.
