Re:Ms. Reyna Sally, aged 57

I am writing to request a daily home visit by a nurse to Ms. Sally who had a fall
last week and require assistance with her personal care.

Ms.Sally admitted into our health care centre last week after injuries to her left
knee,ankle and on her head due to fall from the stairs.Her skin was inflamed and
she had feeling of stinging sensation which is resolved. Due to pain she is
expericing difficulty in walking.

In terms of her social history, she lives alone and do not have any children. Her
neighbours are very helpful and visited her couple of times when she was admitted.

Ms.Sally suffers from diabetis since year 2003.She was hospitalized twice in year
of 2005 due to food poisoning which was resolved after medication within a week.

During dischared, she made a good recovery.However, she requested for personal care
by the nurse due to her mobility problem.Please note that she lives at 198 Harbour
Esplanade, Suite 4059, Docklands, Victoria 3008 and her contact number is 03 9999
8388.
