CERTIFICATION

This is to Certify that an area of ONE THOUSAND SEVEN HUNDRED NINETY


NINE SQUARE METERS (1,799 sq.m) situated at Brgy. San Jose, Rodriguez, Rizal,
owned by New San Jose Builders Inc., may be used by the Municipality of Rodriguez,
Rizal to establish the operation of the Montalban Infirmary Hospital to meet the
public health needs of the communities it serves, operate for the benefit of the general
public and have a beneficial effect upon health care in those communities.

This is to Certify, further, that the beneficial and collaborative relationship between
the NSJBI and the Municipality of Rodriguez shall continue to take effect until the
term of the incumbent mayor, Cecilio C. Hernandez, and may be renewed thereafter
yearly upon mutual consent of both parties to serve the needs of the constituents of
the Municipality.
