This is in reference to the job of tank 312 alloted to M/s Rahul Petrotanks against PO no. 15001206-
OS-48002
Following are the safety violation observed at Tank 312 in 8X4 shift.
1) Person who was carrying out grinding job was not wearing Hand Gloves.
2) Faulty ELCB was being used at site.
Job supervisor Mr. Sayaji Misal was instructed to make necessary corrections to rectify above
deviation.

As per MR SWP ( Safe Work Practice) penalty system B, you are hereby fined Rs 3000/- for non usage
of safe electricity (using faulty ELCB) at site & PPE not being used by worker. In case you fail to submit
fine on time, it will be back charged from your forthcoming challan.
Note : Receipt of letter issued to your supervisor has been attached for your reference
