Rhea Monlore executed an affidavit to correct the birth order of her second son, Dos Monlore, and affirm the correct birth order of her three sons. AFFIDAVIT OF BIRTH ORDER

I, RHEA MONLORE. of legal age, single, and a resident of Zone 4, Brgy


Matiwasay, Tarlac City, after being duly sworn according to law, do hereby depose and
state that:

1. My first son, JUAN MONLORE, was born on February 23, 2006, as appearing
on his birth certificate herewith attached as Annex A;

2. However, the birth order of my second son, DOS MONLORE, born on


November 07, 2007, was erroneously entered as first as appearing on his
birth certificate herewith attached as Annex B;

3. My third son, THIRDY MONLORE, was born on April 07, 2009, as appearing
on his birth certificate herewith attached as Annex C;

4. Wherefore, I am executing this affidavit to attest to the truth of the foregoing


facts as a requisite of my passport application and for the correction of my
second sons birth order.

IN WITNESS WHEREOF, I have hereunto set my hand this 10 th day of May 2017 at
Tarlac City.
