a. Part 2 of the Detail Form found in Appendix A (to
be used as a Front Cover)
b. Initiative Description/Request
c. Receipts/ Invoices/ Relevant Documentation and
Proof of Attendance
d. Student Employment History (provided by ETC)
e. Other Relevant Documentation
f. Declaration of Authenticity.
