Students Initials: A.A.
Age: 10
Grade: 4th grade
Targeted Behavior: Sequencing key events in a story
Intervention: Having A.A. retell a story using pictures in sequential order
Time Period: Starting Jan. 30rd- April 7th
Data Points: Data will be collected up to at least 3 data points. The intervention will last for a
few weeks, in which data will be collected during this time. Once 3 to 4 data points have been
collected, and no progress has been made, then a new intervention will be put into place and new
data will be collected.
Gender: Female
Summary: What did I do? What did I learn?
IEP Objective: The student will be able to correctly sequence 3 key events from a story she has read
getting 3 out of 4 correct.
Data Collection Form: The data collection form will be in a table format. The data will be
collected over the number of sequential events A.A. is able to order. In regards to the chart, there
will be a section for the teacher to mark how many correct and incorrect questions the student
answered. In addition, the teacher will mark how many prompts that were needed for the student
to come to the correct answer.
Baseline: There will be 5 events in each story used. Throughout the intervention, I hope to see an
increase in the number of correct answers, as well as little to no prompts needed for A.A. to
complete the assignment correctly. I will complete a baseline with at least 4 data points.
Intervention: The student will be read a story that has 5 key events within the story. The teacher
will read the story to the student pointing to the picture for each key event that happened in the
story. Once, the story has finished the teacher will mix the pictures around and have the student
place the pictures in order from beginning, middle, and end events in the passage that was read.
After this step has been completed, the teacher can prompt if needed to help student to find the
correct answer. In addition, at this time the teacher can reread the story with the student guiding
them to the correct picture used for that event in the story. The teacher will make note about any
prompts used, or any incorrect events that were placed out of order.
